Hospice Eligibility Criteria

Non-dementia neurological conditions for hospice include ALS, MS, Parkinson’s, Huntington’s Disease, and other neurological conditions.

Hospice Criteria for Neurological Conditions:

  1. Dependence on assistance for 2 or more activities of daily living (e.g. feeding, transferring, ambulation, bathing, continence, dressing)

  2. Structural/functional impairments

  3. Impaired mental function

  4. Increased pain, weakness

  5. Impaired sensory function

  6. Impaired communication

  7. Impaired mobility
