Hospice Eligibility Criteria

Chronic Obstructive Pulmonary Disease (COPD)is a term used for a group of lung diseases that block airflow and make breathing difficult.

Hospice Criteria for COPD:

  1. Palliative Performance Score (PPS) ≤ 70% (see table below)

  2. Dependence on assistance for 2 or more activities of daily living (e.g. feeding, transferring, ambulation, bathing, continence, dressing)

  3. Disabling dyspnea at rest, poorly or unresponsive bronchodilators, resulting in decreased functional capacity.

  4. Progression of end stage pulmonary disease, as evidenced by increasing visits to the emergency room or hospitalizations for pulmonary infections and/or respiratory failure or increasing physician home visits prior to initial certification.

  5. Hypoxemia at rest on room air, as evidenced by pO2 ≤ 55 mmHg or O2 Saturation ≤ 88% on supplemental O2
