End stage kidney disease, also known as renal disease, occurs when the kidneys are so badly damaged that the patient cannot survive without dialysis or a transplant.
Hospice Criteria for Renal Disease:
Palliative Performance Score (PPS) ≤ 70% (see table below)
Dependence on assistance for 2 or more activities of daily living (e.g. feeding, transferring, ambulation, bathing, continence, dressing)
The patient is not seeking dialysis or renal transplant
Creatinine clearance:
< 10 cc/min (< 15 cc/min for diabetics) or
< 15 cc/min (< 20 cc/min for diabetics) with co-morbidity of CHF) or
Serum creatinine > 8.0 mg/dl (> 6.0 mg/dl for diabetics)
